Appointments are scheduled directly with me. Please call me at your convenience (216.283.6852). My office hours are on Tuesdays and Thursdays. I am available for morning, afternoon and evening appointments.

New clients will be asked to complete and sign three forms at their first appointment.

3. Acknowledgement of Receipt of HIPAA Privacy Practices

For your convenience, the new client registration forms are available to download. Click the link below, based on client age, to download and print the appropriate registration packet (requires Adobe Acrobat or Adobe Reader; get a free copy of the latest version of Adobe Reader here).

1. Child or Adolescent Client (under age 18)
2. Adult Client (ages 18 and older)

The registration packet also includes my HIPAA–required Notice of Privacy Practices for your records. Please complete the forms and bring them with you to your first appointment. If you have any questions, please don’t hesitate to call me.
